It doesn’t get more impersonal than a form. This is the first step in clear communications though and a record I can refer to in the future so it’s important.

Before you go to the trouble, let me state my position as an artist and you can decide if I’m the right person to work with.
You must be 18 or older.
I shoot art. I don’t shoot fashion, weddings, kids, dogs, bar mitzvahs, passport, engagements, cute pictures with your boyfriend, or funerals.
It’s not a requirement that you are comfortable with implied nude or nude, but remember, that’s what I typically do with my art. So don’t be surprised if I’m pretty picky about what we shoot outside of that arena. Consider how strange it would be to ask your plumber work on a bad light switch. Or get legal advice from your barista.
Yes, I’m in the self esteem business and I want you to have some art you are proud of. But, remember, I’m only good at what I love to create. You have a comfort zone and so do I. After 1,500 photoshoots I don’t really need another picture of anything in particular for my portfolio. Even as a paying customer, I’m assuming you came to me to see yourself in a photo like I create.
The model release
After filling out the form you are welcome to schedule a shoot (Book an appointment). Once I've had a chance to review the information I will contact you and if everything works out for both of us I will approve the booking and we'll be set.
